Affidavit Regarding Police Records Check

WHEREAS, I acknowledge that in order to safeguard vulnerable populations, the ______, ______ mandates that all ______ students demonstrate the absence of criminal convictions or pending criminal charges by submitting a Canadian Police Records Check Vulnerable Sector Check (VSC).

AND WHEREAS, I have not been able to obtain a Vulnerable Sector Check (VSC) pertaining to my Canadian residence by the program's required date.

NOW THEREFORE, as a substitute for providing such a report, I, ______ of ______ solemnly swear that:

1. I have never faced charges for, nor been convicted of, any offence:

• under the Criminal Code (Canada);

• under the Food and Drugs Act (Canada); or

• under the Controlled Drugs and Substances Act (Canada)

2. I have never been charged with, nor received a pardon for, any sexual offence in any jurisdiction.

3. I have never entered into any undertaking or surety to keep the peace in any jurisdiction.

4. No Restraining Orders have ever been issued against me under either the Criminal Code (Canada) or the Family Law Act (Ontario).

5. There are no warrants or pending charges against me in any jurisdiction.

6. I am making this affidavit solely to temporarily satisfy my program's requirement regarding the absence of criminal or other convictions or outstanding charges, and for no other or improper purpose.

7. I will provide a Police Vulnerable Sector Check to ______, ______ Professionalism Office no later than ______. ______ retains the right to take action based on this check if the result is “not clear” in accordance with its Police Records Check Policy.

8. Should I fail to provide a Police Vulnerable Sector Check by ______, ______ retains the right to treat this as equivalent to a breach of the Professional Behaviour Code of Conduct for Undergraduate Learners, with the associated consequences.
