A Sworn Statement Regarding Police Records Check is a notarized affidavit used as a temporary substitute when your Vulnerable Sector Check (VSC) has not arrived by a program deadline. Universities and colleges in Ontario commonly require a VSC before students can begin clinical placements, field practicums, or co-op terms in healthcare, education, or social services.
The affidavit declares that you have applied for the check, that you have no criminal record to disclose, and that you will submit the official results as soon as they arrive. It is not a permanent replacement for the police clearance.
Vulnerable Sector Checks are the most thorough type of police record screening in Ontario. They cover criminal convictions, outstanding charges, judicial orders, findings of not criminally responsible, and record suspensions for sexually-based offences. Only your local police service can issue a VSC (not third-party providers), and processing times vary widely: some services take four to six weeks, others several months. That gap between police timelines and academic deadlines is exactly why this interim affidavit exists.
This document is most commonly used by students entering supervised placements where they will work with vulnerable populations. Examples include the following.
The form collects the details your program office needs to confirm your identity and the deadline context.
Come prepared so the notary can complete the commissioning in a single visit.
The notary verifies your identity with photo ID, then administers an oath or affirmation. You sign the affidavit while the notary watches, and they complete the jurat with their seal. The signed document goes to your program office as proof of good faith while the official police clearance is being processed.
Virtual commissioning is available under Ontario Regulation 431/20, provided both you and the notary are physically located in Ontario during the session. Notarization at our office is a flat $19.90 per stamp.
The affidavit is a temporary measure only. You remain obligated to submit the official Vulnerable Sector Check once it arrives. Your school will set a secondary deadline for the final document (often 60 to 90 days), and failure to provide it can result in removal from placement. Keep your police service reference number handy so you can follow up on processing status.
