Overview
It is a sworn statement — also called an identity guarantor affidavit or third-party identity affidavit — in which a person who genuinely knows another individual confirms that individual’s identity and details before a notary public or commissioner for taking affidavits.
It is used where someone lacks the usual documents and a trusted person can vouch for them.
It helps confirm identity when standard ID is unavailable.
State how long and how well you have known the person, the particulars you confirm (such as their name and date of birth), and any identification you have seen. Confirm the details are true to the best of your knowledge.
Each institution sets its own rules; confirm a third-party affidavit is acceptable before relying on it.
