Transferring a vehicle in Ontario often comes with a sworn document attached. The most common is the Sworn Statement for a Family Gift of a Used Motor Vehicle (Form 1157E), which lets eligible family members transfer a vehicle without paying retail sales tax — but only if the statement is sworn before a notary public or commissioner of oaths. Other transfers need a sworn statement for the transfer of a used vehicle (for estate or matrimonial situations), a Registrant Identification Number (RIN) letter for corporate vehicles, or an affidavit for a lost or stolen ownership. As an Ontario Notary Public, we verify your identity, administer the oath or affirmation, witness your signature, and apply our seal so Service Ontario accepts the document. We commission the sworn statement; we do not complete the registration itself, which you take to a Service Ontario centre.

Why it matters
Service Ontario relies on sworn statements to prevent tax avoidance and fraud in vehicle transfers. A family gift of a used vehicle can be exempt from retail sales tax, but only when the Sworn Statement for a Family Gift of a Used Motor Vehicle is sworn before a notary public or commissioner of oaths — without that, the exemption is not granted. The same applies to transfers tied to estates or separations, corporate RIN letters, and affidavits for a lost ownership: the sworn-and-sealed statement is what lets Service Ontario process the change with confidence that the facts were declared under oath, with criminal consequences for a false statement. It is a quick step, but skipping it means the transfer stalls at the counter.

Complete the vehicle transfer form or sworn statement but do not sign it yet. Book online or call — same-day slots are usually available.
Present your valid government-issued photo ID. The notary verifies your identity before administering the oath.
Read through the sworn statement and confirm every fact is true — including the family relationship for a gift transfer.
You swear or affirm the statement is true and sign it in front of the notary. Making a false sworn statement is a criminal offence.
The notary completes the jurat, signs, and applies the seal. You take the sworn document to a Service Ontario centre to finish the registration.
