It is a sworn statement — also called a signature verification affidavit or affidavit of genuine signature — in which you confirm your identity and provide a specimen signature that the commissioner witnesses as genuinely yours.
It gives an institution reliable, sworn confirmation that a signature belongs to you.
It is commonly requested for banking, registry, and account matters.
You confirm your full name and identification details, and you provide your specimen signature in front of the notary so it can be witnessed. Bring valid government-issued photo identification to your appointment.
After you fill in your affidavit of identity and signature and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
