Overview
An affidavit of residency — also called a proof of residence affidavit or sworn statement of address — is a written declaration, sworn before a notary public or commissioner for taking affidavits, confirming your home address and the date you began living there.
It is widely accepted where standard documents are unavailable, for example when you live with family, rent informally, or have recently moved.
Organizations often require sworn proof of where you live before they will provide a service or benefit.
A lease, utility bill, or bank statement is the usual proof of address, but not everyone has one in their own name. An affidavit of residency provides a sworn alternative, and a landlord or family member can swear a separate statement confirming you live with them if the organization prefers third-party confirmation.
Once you have filled in your affidavit of residency and downloaded the PDF, do not sign it yet. An affidavit or statutory declaration must be signed in the presence of a notary public or a commissioner for taking affidavits, who administers your oath or solemn affirmation before you sign.
