    A declaration of dependency — also called a proof of dependency affidavit or sworn statement of financial dependence — confirms under oath that a named person relies on you for financial support, and describes the nature of that support.

    It is used where a program or authority needs sworn confirmation that someone is your dependant.

    When you might need one

    Dependency must often be confirmed for benefits and applications.

    • Supporting an immigration or sponsorship application that includes a dependant.
    • Claiming a credit or benefit that depends on supporting another person.
    • Adding a dependant to insurance or workplace coverage.
    • Confirming that a relative relies on you for support.

    What to include

    Identify the dependant and your relationship, describe the support you provide, and state since when. Keep records that show the support, such as transfers or shared expenses, in case the program asks for them.

    How to notarize a declaration of dependency in Ontario

    After you fill in your declaration of dependency and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
