It is a sworn statement — also called a witnessing affidavit or attesting witness affidavit — by the person who saw the signer sign, confirming the signing took place in their presence.
It supports documents where the signature must be shown to have been witnessed.
It is used to prove proper witnessing.
Identify the document, the person who signed, and the date and place of signing, and confirm you were present and saw them sign. For a will, codicil, or other document with its own execution rules, use the matching execution affidavit.
