Overview
It is a sworn statement — also called a change of address affidavit or sworn proof of new address — confirming that you have moved, from where, to where, and when, before a notary public or commissioner for taking affidavits.
It helps where an organization will not update a record without sworn confirmation of your new address.
It is used to update records after a move.
State your previous address, your new address, and the date you moved. Keep any supporting proof, such as a lease or utility bill, available if the organization asks for it.
After you fill in your change of address declaration and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
Minute Notary commissions your change of address declaration online by secure video, under Ontario's remote commissioning rules (O. Reg. 431/20), or in person in Ottawa. Bring valid government-issued photo identification so we can verify your identity before applying the notary seal.
