What is a change of address declaration?
It is a sworn statement — also called a change of address affidavit or sworn proof of new address — confirming that you have moved, from where, to where, and when, before a notary public or commissioner for taking affidavits.
It helps where an organization will not update a record without sworn confirmation of your new address.

What to include
State your previous address, your new address, and the date you moved. Keep any supporting proof, such as a lease or utility bill, available if the organization asks for it.

How to notarize a change of address declaration in Ontario
After you fill in your change of address declaration and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
