It is a sworn statement — also called a declaration of residence or proof of address affidavit — in which you confirm your home address and how long you have lived there.
It is used where you cannot provide a lease, bill, or other document in your own name.
It supports proof of address situations.
State your address, the date you began living there, and your living arrangement (renting, living with the owner, or with family). A landlord can swear a separate confirmation if an organization prefers third-party proof.
