It is a sworn statement — also called a proof of ownership declaration or ownership affidavit — confirming that you own a described item and how you acquired it.
It is used where you must confirm ownership of personal property under oath.

This is for personal property and goods, not title to real estate, which is shown through the land registry. If the property is co-owned, state that and describe each owner’s interest so the declaration is accurate.
