Overview
About this document
- 01
What is an IMM 5604 declaration?
It is the written confirmation, from the parent or guardian who is staying behind, that they do not object to the child immigrating to Canada with the named parent or guardian, and that they understand they may be permanently separated from the child.
IRCC wants the completed declaration together with a photocopy of a valid, readable identity document of the parent who signs it.
- 02
When you need one
IRCC asks for it when:
- a child is immigrating to Canada with one parent or guardian and the other is not coming;
- the parent staying behind is a former spouse or former common-law partner of the parent who is immigrating;
- the child is travelling to Canada with someone other than a parent or guardian, which the form records separately.
- 03
What to include
Have these details ready:
- Your full name and address.
- The child's full name and date of birth, exactly as on the passport or travel document.
- The full name of the parent or guardian the child is immigrating with, and their relationship to the child.
- An emergency contact telephone number.
- The place where you will sign.
- 04
Official source and who accepts it
IRCC prescribes IMM 5604 and expects its own form, printed and signed by hand. Download the current version from canada.ca, fill it in with the details you prepare here, and bring it unsigned to the appointment.
The witness section must be completed by a notary public: their title and full name in block letters, their professional stamp, their signature and the date. Because the notary witnesses the signature itself, it is signed in person, not by video.
- 05
Common mistakes
These are the usual reasons IRCC sends it back:
- Signing before the appointment; the form must be signed in front of the notary.
- Putting two children on one form; IRCC wants one declaration per child.
- Names or dates that differ from the child's passport or travel document.
- Forgetting the photocopy of the signing parent's identity document.
- 06
How to have it witnessed in Ontario
Fill in the form on this page to prepare the details, then copy them onto IRCC's IMM 5604. Do not sign it yet: the parent or guardian who is staying behind signs it in front of the notary.
Book an in-person appointment with Minute Notary. Bring the printed form unsigned, your original identification, and a photocopy of that identification for IRCC. The notary watches you sign, then completes the witness section with their name, stamp, signature and the date.

Frequently asked
Questions about the Declaration from Non-Accompanying Parent or Guardian (IMM 5604)
- Can IMM 5604 be signed by video?
- No. The form needs a notary public to witness the signature, so the parent signs in person in front of the notary.
- Can a commissioner for taking affidavits witness it?
- The form names a notary public as the witness, so use a notary public rather than a commissioner who is not a notary.
- The parent staying behind lives outside Canada. What then?
- They sign in front of a notary public where they live. Our notaries can witness it only in person in Ontario.
- Do I need one form per child?
- Yes. IRCC asks for a separate IMM 5604 for each child immigrating without both parents or guardians.
- What identification should the parent bring?
- Their original identity document to show the notary, and a photocopy of a valid, readable identity document to send to IRCC with the form.
