REVOCATION OF POWER OF ATTORNEY
I, ______, of ______, make this revocation.
On ______, I made ______ naming ______ as attorney (the “earlier power of attorney”).
I revoke the earlier power of attorney in full, with every authority it gave. If it named a substitute attorney, that appointment is revoked as well.
I make this revocation in writing and sign it in the presence of two witnesses, as the Substitute Decisions Act, 1992 requires.
I direct each attorney named in the earlier power of attorney to stop acting under it and to return to me the original and every copy they hold.
I will give a copy of this revocation to each attorney named in the earlier power of attorney, and to every person or institution that I know holds a copy of it or has relied on it, such as my bank or my health care providers.
Signed at ______________________, Ontario, on ______________________.

Signed by the grantor in our presence. We have each signed below as witnesses, in the grantor's presence.
