This affidavit is a sworn statement confirming that you are raising one or more dependent children on your own and that no spouse's income should factor into your OSAP funding calculation. Your school's financial aid office uses it to classify your application correctly so OSAP assesses your need based on your income alone, rather than assuming a two-parent household.
The document carries legal weight because you make it under oath before a notary public or commissioner for taking affidavits. An unsworn copy will not satisfy the financial aid office.
OSAP has a specific definition. You must meet all of the following criteria to be classified as sole-support.

You can swear the affidavit in person at our Ottawa office or virtually under Ontario Regulation 431/20 (both you and the commissioner must be in Ontario during the video call). Either way, have the following ready.
At your appointment, the notary or commissioner will verify your government-issued photo ID, confirm that you understand you are making a statement under oath, and watch you sign the document. They then complete the jurat, which is the clause recording where, when, and before whom the oath was taken, and apply the official seal.
Notarisation at Minute Notary is a flat $19.90 per stamp. This affidavit typically requires one stamp. The entire process takes only a few minutes once the form is complete.
Swearing a false affidavit is perjury under section 131 of the Criminal Code of Canada, an indictable offence carrying up to 14 years' imprisonment. Beyond criminal consequences, a false statement can result in the loss of your OSAP funding and a requirement to repay amounts already received. Make sure every detail is accurate before you sign.
If your marital status changes after you submit the affidavit (for example, you enter a common-law relationship or remarry), you are required to report the change to your financial aid office. Your sole-support classification may be reassessed and your funding adjusted.
