OSAP calculates a student's financial need partly by looking at what their parents earn. When that income was earned in another country or comes from non-taxable sources in Canada, the CRA has no record of it and OSAP cannot pull it automatically. In that situation, the student's financial aid office asks the parent to swear an affidavit declaring their income so the application can proceed.
This is the general parental income affidavit. A separate, more detailed version exists for cases where the school needs specific dollar figures, currency conversions, and arrival dates. Your financial aid office will tell you which form they require.
The financial aid office will request this document when parental income cannot be verified through CRA records. Common situations include the following.
The form collects the details needed to identify you and tie the affidavit to your child's OSAP file.
Only the parent whose income is being declared needs to swear the affidavit. If both parents have foreign or non-taxable income, both can be included on a single form. The template has space for a second parent's name and date of birth. Each parent who is declaring income must be present (in person or virtually) to swear or affirm before the commissioner.
The affidavit has no legal weight until sworn or affirmed before a notary public or commissioner for taking affidavits. At the appointment, the notary verifies your government-issued photo ID, confirms you understand you are making a statement under oath, watches you sign, and completes the jurat.
Virtual commissioning is available under Ontario Regulation 431/20, provided both you and the commissioner are in Ontario during the video call. Notarisation at Minute Notary is a flat $19.90 per stamp; this affidavit typically requires one stamp.

Most schools require supporting documents at least 40 days before the end of the study period. If you do not provide the affidavit, your child's OSAP application will remain incomplete and funding cannot be released.
Swearing a false affidavit is perjury under section 131 of the Criminal Code of Canada, an indictable offence carrying up to 14 years' imprisonment. Ensure the information is accurate before you sign.
