It is a sworn statement — also called a lost ID affidavit or declaration of lost or stolen card — confirming that an identity document has been lost or stolen.
Issuers often require it before they will cancel the original and provide a replacement.
It supports replacing a missing identity document.
Describe the document and its number if known, who issued it, and how and when it was lost or stolen. Where a document was stolen, you should report it to police; the declaration can record the report number but does not replace a police report.
