A statutory declaration of marital status — also called a marital status affidavit or sworn statement of marital status — confirms your relationship status under oath before a notary public or commissioner for taking affidavits.
It is used where your status affects a benefit, a tax filing, a pension, or how an organization records your file.
A sworn statement of marital status is requested in many benefit and administrative situations.
State your current status accurately and the date it began where relevant. If you are separated or in a common-law relationship, an organization may ask for more detail, such as the separation date or how long you have cohabited, so be ready to explain.
