What is the DS-3053 form?
DS-3053 is the U.S. Department of State’s Statement of Consent, in which a non-applying parent or guardian consents to the issuance of a U.S. passport to their child.
It is used when both parents cannot appear together, and it must be signed in front of, and notarized by, a notary.

It is used for a child’s U.S. passport application.
- One parent or guardian cannot appear in person for the child’s application.

Who signs it, and where it goes
The parent or guardian who will not be present at the passport appointment signs the DS-3053, in front of the notary, showing government-issued photo identification; a photocopy of the front and back of that identification travels with the form. Because a U.S. federal agency is the receiver, sign it in person unless the passport agency handling the application confirms it will accept a remotely notarized statement.
The signed statement goes with the child’s DS-11 application, which the other parent submits in person at a U.S. embassy or consulate when applying from Canada, or at a passport acceptance facility in the United States. It must be notarized within 90 days of the application, so time the appointment to the passport visit rather than signing months ahead.
