Overview
It is a sworn statement — also called a lost ownership affidavit — confirming that the vehicle permit, the document that shows ownership of your vehicle, has been lost or stolen.
A registry may ask for it before reissuing the permit, to confirm the original is genuinely gone.
It supports replacing a missing ownership permit.
Describe the vehicle (year, make, model, VIN), the plate and permit number if known, and how and when the permit was lost or stolen. Confirm it has not been transferred, sold, or surrendered.
After you fill in your lost vehicle permit declaration and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
Minute Notary commissions your lost vehicle permit declaration online by secure video, under Ontario's remote commissioning rules (O. Reg. 431/20), or in person in Ottawa. Bring valid government-issued photo identification so we can verify your identity before applying the notary seal.
