About this document
- 01
What is an acknowledgement of debt?
It is a sworn statement — also called a debtor’s acknowledgement or admission of debt — in which the debtor confirms the debt and may set out repayment terms.
It gives the creditor a clear, sworn record of the amount owed.
- 02
When you might need one
It is used to confirm and commit to repaying a debt.
- A creditor asks you to confirm, under oath, that you owe a debt.
- You are agreeing to a repayment plan and want to record it.
- A debt must be acknowledged before a settlement or arrangement.
- 03
Good to know
Acknowledging a debt can confirm liability and may affect limitation periods. Because the consequences can be significant, consider legal advice before signing if the amount is large.

How to notarize a acknowledgement of debt in Ontario
After you fill in your acknowledgement of debt and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
Minute Notary commissions your acknowledgement of debt online by secure video, under Ontario's remote commissioning rules (O. Reg. 431/20), or in person in Ottawa. Bring valid government-issued photo identification.
