What is a debt repayment declaration?
It is a sworn statement — also called an affidavit of debt paid or declaration of debt satisfied — confirming that a debt you owed has been repaid in full, with no balance remaining.
It gives proof of payment where you cannot obtain a written release from the creditor.

When you might need one
It is used to close out and prove repayment of a debt.
- Proving a personal loan has been fully repaid.
- Releasing a guarantor or a lien once the debt is cleared.
- Providing a lender or registry with sworn confirmation of payment.

What to include
Identify the creditor, what the debt was for, the amount, and the date it was repaid in full. A creditor’s written release is stronger where you can obtain one, but this declaration helps where you cannot.

How to notarize a debt repayment declaration in Ontario
After you fill in your debt repayment declaration and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
