An Affidavit of Name Change is a sworn statement declaring that you have legally changed your name and that your former name and current name both refer to the same person. It bridges the gap between old records and your current legal identity, giving institutions the sworn proof they need to update their files.
This affidavit does not create a name change. It confirms one that has already happened, whether through ServiceOntario, by marriage, by divorce, or through a foreign process.
Many Canadian institutions accept an Ontario change of name certificate on its own. However, a sworn affidavit is often required when the certificate alone is not enough for the receiving party.
Ontario law provides several paths to a legal name change. This affidavit applies regardless of which path you took.
