It is a sworn statement — also called a self-employed proof of income affidavit — confirming the nature of your business and the income you earn from it.
It helps where, as a self-employed person, you cannot provide an employer’s pay stubs or a standard employment letter.
It is used wherever self-employed income must be confirmed.
Describe your business or self-employment, how long you have run it, and your income with the period it covers. Keep supporting records — invoices, bank statements, or tax filings — available, as they are often requested.
