    A declaration of relationship — also called a proof of relationship affidavit or sworn statement of relationship — confirms under oath the family or personal connection between you and another person.

    It is often used where official documents alone do not fully establish the relationship, or where an authority asks for a sworn statement in support.

    What to include

    Name both people, state the exact relationship, and explain how you know the facts to be true. Where helpful, refer to documents that support the relationship, such as birth or marriage records, and keep them available if requested.

    How to notarize a declaration of relationship in Ontario
