An Affidavit of Vehicle Ownership is a sworn statement in which you declare under oath that you are the lawful owner of a specific motor vehicle. It is used when original proof of ownership (the signed bill of sale, registration permit, or vehicle portion of the ownership) has been lost, destroyed, or was never properly completed at the time of purchase.
The affidavit does not replace a vehicle registration; it serves as supporting evidence that ServiceOntario can consider alongside other documentation when processing a transfer or registration.
This situation arises more often than people expect. You may need a sworn statement of vehicle ownership in any of the following circumstances.

Arrive prepared so the notary can complete the commissioning in a single visit. You will need the following.
The notary verifies your identity using government-issued photo ID, then administers an oath or affirmation. You sign the affidavit in front of the notary, who completes the jurat (the certificate recording the date, location, and who administered the oath) and applies their notarial seal. The signed and sealed document can then be presented to ServiceOntario.
Virtual commissioning is available under Ontario Regulation 431/20, provided both you and the notary are physically located in Ontario during the session. Notarization at our office is a flat $19.90 per stamp.
ServiceOntario assesses each case individually. In addition to the sworn affidavit, they may ask for a Used Vehicle Information Package (UVIP), which shows the vehicle's registration history and lien status. The UVIP costs $20 and can be ordered online or picked up at a ServiceOntario centre. Call your local office before your visit to confirm exactly what documentation they need for your situation.
Swearing a false affidavit is perjury under section 131 of the Criminal Code of Canada, an indictable offence carrying up to 14 years' imprisonment. Beyond criminal liability, any vehicle transfer that relied on a false sworn statement could be reversed, and the vehicle may be seized if it is found to be stolen or otherwise encumbered.
