It is a sworn statement — also called a child identity affidavit or parental affidavit of identity — in which a parent or guardian with personal knowledge confirms a child’s identity and particulars before a notary public or commissioner for taking affidavits.
It is used where an institution needs sworn confirmation of who a child is.

State your relationship to the child, the child’s full legal name, date and place of birth, and any other names the child uses. Confirm the particulars are true to the best of your knowledge.
