It is a sworn statement — also called a true likeness affidavit or photo identification affidavit — confirming that an attached photograph genuinely shows the person named.
It is used where an application requires a photo to be sworn to as an accurate likeness.
It supports identity and travel applications.
The notary witnesses the sworn statement and may mark the photograph as an exhibit. Bring the photograph and valid government-issued photo identification to your appointment, and follow the receiving body’s photo specifications.
