It is a sworn statement — also called a hardship affidavit or declaration of inability to pay — setting out your financial circumstances under oath.
Courts, agencies, and institutions may accept it to support a fee waiver, a payment arrangement, or other relief.
It supports requests for relief based on your finances.
Describe your circumstances, your approximate income, the dependents who rely on you, and your major expenses or debts, and state clearly the relief you are requesting. Keep supporting records available, as the recipient may ask for them.
After you fill in your financial hardship declaration and download the PDF, do not sign it in advance. A sworn document must be signed in front of a notary public or commissioner for taking affidavits, who administers your oath or solemn affirmation first.
