It is a sworn statement — also called an acknowledgement of receipt of payment or proof of payment declaration — confirming under oath that funds were received.
It is used where formal proof of a payment is needed.
It supports proof of payment and settlement.
State who paid you, the amount, how it was paid, the date, and what the payment was for, and whether it is full and final or partial. Choose "full and final" only if that is truly the case, as it can affect later claims.
