  1. 01

    What is a statutory declaration of residence for school registration?

    It is a solemn declaration by a parent, guardian or adult student confirming the student's home address, how long the student has lived there, and who owns or rents the home.

    It fills the gap when the bills and the lease are in another person's name, for example a grandparent, another relative, or a landlord who rents you part of a house.

  2. 02

    When you need one

    Families are often asked for this declaration when:

    • you live with relatives who own or rent the home;
    • you rent a room or a basement unit and your name is not on the main lease;
    • you have just moved and the bills are not in your name yet;
    • the board asks for more proof because the address decides which school the student attends.
  3. 03

    What to include

    Have these ready:

    • each student's full name, date of birth, and school and grade or program;
    • the full home address and the date the student started living there;
    • the name of the person who owns or rents the home and your relationship to them;
    • why the usual documents are not in your name;
    • the name of the school board, college or university that will receive it.
  4. 04

    Official source and who accepts it

    Ontario's enrolment register instructions require each school board to review documents that support a pupil's residency, such as a current utility bill, property tax bill or home phone bill, and to record which ones it reviewed.

    The board, college or university decides which documents it accepts. This declaration is one it may take alongside them; it does not oblige the board to accept it.

  5. 05

    Common mistakes

    Avoid these:

    • Using a relative's address where the student does not actually live. Boards check, and a false declaration is a criminal offence.
    • Leaving out a brother or sister who is registering at the same time.
    • Not bringing the owner's or tenant's own document when the board asks for one.
    • Forgetting to tell the school when the student moves.

    How to get it notarized in Ontario

    Fill in the form on this page and download a clean, ready-to-sign PDF. Do not sign it yet: an affidavit or statutory declaration is signed in front of a notary public or commissioner for taking affidavits, after the oath or affirmation.

    When you are ready, book an appointment with Minute Notary. You can swear or affirm it before a notary by video from anywhere in Ontario, or in person on a mobile visit. Bring government-issued photo ID; the notary administers the oath or affirmation and applies the seal.

Frequently asked

Questions about the Statutory Declaration of Residence for School Registration

01
Will the school board accept this declaration?
It depends on the board. Each school board, college or university sets its own proof-of-residence rules and decides what it accepts. Check its registration page or ask the school before your appointment.
02
Who signs it, me or the homeowner?
The parent or guardian registering the student signs it, or the student if 18 or older. Some boards also want a letter or declaration from the owner or tenant; ask whether yours does.
03
Can I list more than one child?
Yes. Add each student who lives at the address, with the school and grade for each.
04
Can I swear it by video?
Yes. A statutory declaration can be commissioned by video under Ontario's remote commissioning rules, from anywhere in Ontario, or in person on a mobile visit.
05
What should I bring to the appointment?
Government-issued photo ID, the completed declaration unsigned, and any document the board asked you to provide with it.
