When you might need one
OSAP and the other student-aid programs accept a sworn declaration when your circumstances do not fit the online application: a marital or common-law status the system cannot verify, sole-support parent status, or income earned abroad or not taxed in Canada. The financial-aid office tells you which statement it needs and its deadline; the wording in these templates follows what those offices ask for.
Students usually commission the declaration online by video, which suits a campus schedule and a deadline that is days away. Have the supporting document ready to show on the call — the tax slip, the lease, the letter from the program — and sign only when the commissioner asks. OSAP accepts a declaration commissioned by video under O. Reg. 431/20; a college or university office with its own form will say so on the form.
