What is a Statutory Declaration of Name Discrepancy?
It is a sworn declaration in which you confirm that the different spellings or orderings of your name on your identity documents all refer to you.
The declaration does not change your legal name. It records, under oath, that the variations already on file describe one person.

When you might need one
Institutions commonly ask for this declaration when:
- Your passport spells your name differently from your birth certificate.
- Your given name and surname appear in a different order across documents.
- A transliteration from a non-Latin script produced more than one spelling.
- A bank, employer, school, or immigration authority flags the mismatch.

What you'll need to fill it in
Have the following ready before you start:
- Both documents that show the differing names, so you can copy each name exactly.
- Your date and place of birth.
- A short, factual explanation of how the difference arose.

Copy each name exactly as printed
Transcribe each name character for character from the document itself, including middle names, hyphens, and accents. A declaration that paraphrases the name is far more likely to be rejected than one that reproduces it.

- Does this change my legal name?
- No. It only records that the existing variations refer to the same person. A legal name change in Ontario is a separate application to ServiceOntario.
- Can I list more than two versions of my name?

- Will the receiving institution accept it?
- Most do, but requirements vary. Check whether the institution has its own form or specific wording before your appointment, because a notary commissions the document you bring rather than choosing its contents.
