Court Services publishes Form 16B as a Word file only
Unlike most court forms, there is no official PDF of Form 16B — the forms table lists a Word document and marks the PDF column not applicable. Its current revision is 1 February 2021. Download the Word file, complete it, and print it unsigned for the appointment.
Because there is no PDF to fall back on, be careful not to reuse a copy someone has already edited: open a fresh download each time so the revision stamp at the foot of the form is current.

Which form belongs to which proceeding
Ontario prescribes a different affidavit of service for each track, and filing the wrong one is a wasted trip. Form 16B is for an ordinary civil action under the Rules of Civil Procedure. Form 8A is Small Claims Court. Form 6B is family court. Form 146A is the Federal Courts.
As with Small Claims, a sworn affidavit is not always required: where a lawyer served the documents, Form 16B.1 — the Lawyer's Certificate of Service — takes its place.

Questions about the Affidavit of Service (Form 16B) — Ontario Rules of Civil Procedure
- When do I use Form 16B instead of Form 8A?
- Form 16B belongs to an ordinary Superior Court of Justice action governed by the Rules of Civil Procedure. Form 8A belongs to the Small Claims Court under O. Reg. 258/98.
- Where is the PDF of Form 16B?
- There is not one. Ontario Court Services publishes Form 16B as a Microsoft Word file only; the PDF column in its forms table is marked not applicable.
- My lawyer served the documents. Do I need Form 16B?
- Probably not. A lawyer may file Form 16B.1, the Lawyer's Certificate of Service, instead of a sworn affidavit.
- Can I swear it over video?
- Yes. An Ontario notary can commission it remotely under O. Reg. 431/20. Confirm the registry's current practice before filing.
