Overview

When you might need one

IRCC's own forms govern where they exist: IMM 5409 for a common-law union, IMM 5646 for the custodian of a minor student, IMM 5604 for a parent who stays behind. Those are declared on the department's form; our page prepares the facts and shows what the form asks. A translator's affidavit, a name-discrepancy declaration or a statement of absences has no prescribed form, and the templates here are written for that purpose.

Most of this group is commissioned online by video under O. Reg. 431/20 — the declarant shows government photo ID on the call and signs when the commissioner asks. The exception is a form the department wants witnessed rather than sworn, which is signed in person. An invitation letter is not sworn at all; a notary witnesses the host's signature when the visa office or the visitor asks for one, and that is an in-person step.

  • 01Declaring a common-law relationship on IMM 5409 or in a general declaration
  • 02Inviting a parent, a relative or a friend to Canada and supporting their visitor visa
  • 03Swearing a translation, a name discrepancy or absences from Canada for IRCC
