Passports, travel & children

Documents for a passport application and for a child who travels or stays with someone else: the declaration Passport Canada accepts when no guarantor is available, consent letters for a minor crossing a border, and the written authority a caregiver or a clinic needs. The passport declaration is sworn online by video; a consent letter is a witnessed signature, signed in person.

Overview

When you might need one

Passport Canada's statutory declaration in lieu of a guarantor is a prescribed form (PPTC 132, or PPTC 326 for a travel document) sworn before a commissioner for taking affidavits, a notary public or a justice of the peace. It is commissioned online by video under O. Reg. 431/20 and the sealed form goes into the application. The DS-3053 consent for a U.S. child passport works the same way for the U.S. Department of State.

A child travel consent letter is recommended, not required by Canadian law; border officers and airlines ask for it when a child travels with one parent, a relative, a group or alone. It is a witnessed letter, not a sworn one, so the parent signs it in person in front of the notary. The same is true of a caregiver authorization or a consent to medical treatment: prepare it online, bring ID, and sign in the room. Check the destination country and the carrier for their own wording before you travel.

  • 01Swearing the PPTC 132 declaration when a passport applicant has no eligible guarantor
  • 02Sending a child abroad with one parent, a relative, a group, or alone
  • 03Giving a caregiver written authority a school or a clinic will accept, including consent to medical treatment
