General affidavits & sworn statements

The general affidavit and statutory declaration, plus the sworn statements that fit no single office: identity, residency, a lost document, a late filing, a witnessed signature, an address history. This is the largest group and the one to start from when a receiving office simply says it needs the statement sworn.

Statutory declarations

Overview

When you might need one

An affidavit is evidence sworn or affirmed under oath; a statutory declaration is a solemn statement of fact under the Canada Evidence Act with the same force as an oath. A bank, an insurer, a school, a registry or an employer asks for one when a plain letter is not enough. When the receiving office has its own form, use theirs and treat these templates as a guide to the wording; the general and custom templates here are for the many cases where it does not.

Every template in this group is free to fill in and download. The paid step is the commissioning: you sign in front of the notary, on a video call under Ontario's O. Reg. 431/20 or in person, after showing government photo ID. Do not sign in advance, and bring whatever the statement refers to — the expired card, the lost permit, the letter you are answering — so the notary can note it and mark an exhibit if one is needed.

  • 01Putting any fact on the record in a general or joint affidavit when no prescribed form exists
  • 02Confirming your identity, your address, or a lost or missing document
  • 03Swearing that you witnessed a signature, or explaining a delay a registry or an employer will not accept in a letter
